Step 9. Attend Hearing, if necessary. You should contact your court to determine if a hearing will be held on your name change request and, if scussed in Step 4 will also be useful to you in this filing. Bring a selfaddressed, stamped envelope to court with you so that, if necessary, the court can mail the Notice of Hearing back to you and ying, copies of the newspaper ads, any additional documents given to you by your newspaper, as well as at least 3 copies of each of these documents, to the court with you to be filed. The procedures di Instructions-Name Change Kit 7 After the 30-day period has passed, bring your original Affidavit of Publication, Notice of Filing of Proof of Publication and Request for Hearing, the Notice of Heardays before filing your documents with the court. Complete Affidavit of Publication and Notice of Filing of Prdize your personal safety. If this is a concern for you, be sure to explain this to the judge at your hearing or, if there is no hearing, contact the clerk of your local court to explain your situatithey will need from you. NOTE: There is a possibility that you may be exempted from the publication requirements discussed, if the court determines that the publication of your name change might jeopaave forms that you may use to create this publication notice. You may want to contact your local newspapers to see if they have templates that they require, or if there is additional information that detail in Step 8 below. Different newspapers may require more time to process and complete your submission than others. If you wait too long, your name change request may be delayed. Many newspapers hange to publish a notice of your name change for the required period. Be sure to save a copy of each week's printing of your notice; you will need to file this with the court, as explained in greater st publish notice of your name change at least once a week for three consecutive weeks in a newspaper of general circulation in your county.
