This may take some time, so be patient! A list of agencies that you may wish to contact, and contact iout $10 apiece. You may wish to request copies of this order at the time of your filing, forms for which may be obtained at your local court. Step 11. Changing your Personal Documents. After you have our Order for Name Change and mail it to you. This is the final official step of the name change process. Contact your local court to obtain certified copies of this order, which will probably cost abur name change to any organization or person specifically ordered by the court. Step 10. Getting your Certified Order. After your hearing, if you have previously requested it, the court will certify y and Naturalization Service, the Selective Service System, a former spouse, the Bankruptcy Court, or any other party that the court considers appropriate. In any event, be sure to provide notice of yo Your Name Change. In connection with your application, the judge may require that you notify additional specific parties of your name change request. This may include such entities as the Immigration of your name change to other parties, as explained in Step 9. The judge will probably sign the document at this time and grant your name change. C. FINISHING UP. Step 9. Notify Additional Entities ofhappen. If a hearing is held, and the Order wasn't previously signed, the judge will probably sign the document at that time and grant your name change. In addition, he may order you to provide noticelerk and wait for your case to be called. At the hearing, make note of and comply with all instructions given by the judge. Don't be nervous ­ the judge will guide you through any proceedings that do u need to bring TWO ADULT WITNESSES with you to your hearing. These should be individuals that know you well and can testify to your identity. When you arrive in the courtroom, give your file to the cg all relevant papers with you, including the file-stamped Application, the Order (if the clerk did not take it from you at the time of filing) any other items that you think might be needed. Appear in Court If there is a hearing, be sure that you know where the courtroom is and be certain to brinl filing. Alternatively, you may find the name of your judge online at the following site: http://www.courts.state.mn.us/page/?pageID=108 If you have not received a response from your court within 2 wce, contact your judge's clerk to schedule your hearing. To get the number for your judge and his clerk, you may want to ask the clerk Instructions-Name Change Kit 9 who assisted you in your initiaimately 2 weeks from the time of your filing, a "Notice of Judicial Officer Assignment" will be sent to you, which will inform you of the judge assigned to your case. After you have received this Notiuest, or other specific questions about your case, you can use this number to allow court personnel to assist you more efficiently. Step 7. You Will Receive a Notice, and Schedule your Hearing. Approxer assigned to your request for a name change. This is the reference number that you will use for the duration of your time in court. If you have any questions about the status of your name change reqe with your documents ­ you should contact the court to determine the exact amount required. Step 6. The court will assign a court file number and establish a file. The "court file number" is the numblf-addressed and stamped envelopes with your documents, so that the clerk can return both a file-stamped copy of the forms and, where possible, the signed order. Do not forget to include the filing fe schedule that hearing. Additionally, you should ask that if the order can be signed without a hearing, that the clerk return the executed order to you in one of the envelopes provided. Include two sebmitting. Your letter should request that the court file the necessary forms and that the judge execute the order. You should also state that, if a hearing is needed, the letter serves as a request tobe accepted. Mail ­ If you decide to mail your forms, you should include a cover letter that explains that you are requesting a name change and that includes a list of all of the forms that you are suents only: personal checks and credit cards will not be accepted in these jurisdictions. You should contact your local court before going to the courthouse so you will know what forms of payment will You will be required to pay the filing fee at the time you bring in your documents, which will be around $245. Note that many Minnesota courts require either cash, money order, or certified check paym not be official until the judge has signed your order, as discussed below. You can contact your court in advance to determine where you will need to file the documents, or can ask at the courthouse. re filing. In any event, you should listen carefully to the clerk's instructions and follow them precisely. NOTE: Even though you have completed the filing of your request to change your name, it will the Order with you to a court hearing, at which time the judge would complete and sign it. In Person ­ You should bring in the original copies of your completed NG TO COURT. Step 5. File the signed and notarized Application with your district court. You can file the forms in person, or you may mail in your completed documents. It is usually a better idea to gr of public record, the requirement that your petition be made under oath may be waived by the court. You should consult with your local clerk if you believe that this describes your situation. B. GOIion notarized at the time of your filing (see Step 5). NOTE: If you have reason to believe that a serious threat to your or your family's health or safety may exist if your name change becomes a mattetions, your signature may be acknowledged by a clerk, a deputy clerk or a notary public. You should determine if this is the case in your jurisdiction, in which case you may be able to have your petitses, or available in close proximity to those buildings. You may contact your local court for more information, or locate a nearby notary public in your local phone book. In addition, in some jurisdicn them in his/her presence. Once you have done this, your petition becomes what is known as a "Verified Application" or a "Notarized Application." In many areas, there are notaries located in courthou will sign and date the Order granting your name change. Step 4. Sign the Application in front of Notary Public. After you have completed your documents, you must bring them to a Notary Public and sig COMPLETE THE INFORMATION IN THE NEXT ITEM; this is for the judge to complete. DO NOT SIGN OR DATE THE ORDER IN THE JUDGE'S BLANK.
