Revocation of Power of Attorney (Canada)

If the donor (the person granting a power of attorney) decides to revoke the document, it is almost always required that the revocation be in writing. This form is to be sent by the donor to the attorney informing them that the power is revoked and confirming all actions previously taken by the attorney.

This form can be used in the following provinces: Alberta, British Columbia, Manitoba, New Brunswick, Newfoundland and Labrador, Northwest Territories, Nova Scotia, Nunavut, Ontario, Prince Edward Island, Saskatchewan and Yukon.
