- Parties: Sets forth the names and addresses of both the prime contractor and the team member (subcontractor);
- Customer Information: Sets out the name of the customer, number of request for proposal and a detailed nature of the project;
- Relationship of the Parties: Sets out that prime contractor will submit the proposal and team member will be assigned a certain portion of the work as specifically set out;

- Publicity: Sets out that neither party will issue any news releases or announcements regarding their involvement in this project unless written approval is received.
- Description of Work: Schedule A which sets out a detailed description of the work assigned to the team member and prime contractor.
