This Living Will Forms for use in Vermont allows a competent adult to direct the providing, withholding, or withdrawal of life-prolonging procedures in the event that such person has a terminal condition, has an end-stage condition, or is in a persistent vegetative state.

(Terminal care document) A person of sound mind who is 18 years of age or older may execute at any time a document commonly known as a terminal care document, directiinued with dignity and without pain.
