What is a Waiver and Assumption of Risk?

A Waiver or Assumption of Risk form is a document generally used by a business to inform a customer of the risks involved with a certain activity. By signing the form, the customer agrees to assume the risk of the activity and to waive any right to file a claim in the event of injury or other harm. This type of form is sometimes used when one is engaged in dangerous recreational activities such as skydiving, scuba diving, skiing etc.
