This Minor Release is written consent by the parent or legal guardian of a minor child given to another which grants permission to take photographs or record the voice of the child in connection with a motion picture. This Release sets forth the express terms of the agreement and further warrants that the child was a minor at the time of signing. A written Minor Release is important in the event there are future disagreements between the parties regarding the minor child.

This Minor Release contains the following provisions:

• Parties: Sets forth the name of the parents or legal guardian of the minor child and to whom permission is granted to take the photographs or record the child’s voice;
• Picture: The title of the motion picture for which the child’s photographs or voice will appear;
• Consent and Permission: The parent or legal guardian of the minor child agree and warrant that the child will not disaffirm or disavow this consent on the grounds he/she was a minor nor will the child recover any sums for appearing in the picture unless specified in a Child Actor Employment Agreement;
• Signatures: The parents or legal guardians of the minor child must sign and date this release.
