This Change Order will effectively set forth in writing any changes made to an original contract. This change order sets forth the name of the original agreement (i.e. building contract) and the parties to the original contract. It also sets out a brief description of the changes to the work or services to be performed and if the contractor will receive additional compensation for the changes. It is crucial that any changes to an existing contract be clearly and succinctly set out in writing. A written Change Order will prove invaluable in the event there are disagreements or misunderstandings between the parties about the changes or revisions made to an existing contract.

  • Original Contract/Parties: The title of the original contract and names of the parties to this contract;
  • Changes: Sets forth in detail the changes made to the existing contract;

This is a Change Order to the Name of Original Agreement between Parties (ie. Building Contract) dated Date of Original Contract (the "Original Contract") between Name of Contractor (as in Original Contract) (the "Contractor") and Name of Principal (as in Original Contract) (the "Principal").
 
1.   The parties hereby agree that the following change(s) (the "Changes") shall be made to the services to be provided by the Contractor in the Original Contract:
 
Insert brief description of change to work and services to be provided by Contractor
 
 
2.   As consideration for the Changes, the Principal shall pay to the Contractor the additional sum of Cost of Requested Change (ie. $500.00), which shall be paid Specify when Payment of Change will be made (ie. concurrently with the signing of this Change Order, or upon completion of the work or services to be performed under the Original Contract, or on July 31, 2001)
 
3.   The parties hereby confirm that all other terms and conditions of the Original Contract are in full force and effect, unamended except as expressly provided in this Change Order or in any other Change Order agreed to in writing between the parties hereto.
