An appraisal affidavit provides the basic details of an appraisal that is signed and sworn to before a notary public.  This type of document is often required by financing institutions at the closing of a real estate transaction.  This document supplies essentially the same information as the basic Appraisal Report, but in a more formal affidavit.  To complete this form, the following needs to be completed:

1)   Date of appraisal
2)   Name of appraiser
3)   Address of appraiser
4)   Name of person requesting appraisal
5)   Address of property appraised
6)   Legal description of property appraised
7)   Market value of land only
8)   Market value of improvements to land
9)   Total market value
10)   Qualifications of appraiser
11)   Signature of appraiser
12)   Printed name of appraiser

The following must be completed by a Notary Public:

13)   State where Notary Public is located
14)   County where Notary Public is located
15)   Date Notary Public signed
16)    

Name of owner (Seller) of property
17)   Signature of Notary Public
18)   County where Notary Public is located
19)   State where Notary Public is located
20)   Date that Notary Publics commission expires
